The Christian Science periodicals occasionally have carried notices requesting branch churches, societies, and college organizations to send to the Archives of The Mother Church historical accounts relating to the inception and growth of their organizations. Many branches of The Mother Church already have responded to these requests; others have not yet done so. We, therefore, are taking this occasion to invite all branches which have not yet sent this material for our permanent files to do so at this time. The history should be dated and signed by the clerk. A photograph of the building in which services are held should accompany the history. A picture eight by ten inches in size will meet the requirements. The history should include the following information:
Date of first interest in Christian Science
Circumstances connected with this interest
Full names of earliest students
Full names of earliest practitioners
Date of first meeting or service
Date of organization
List of charter members
Date of recognition by The Mother Church
Date of first lecture
Date of purchase of church property
Date of erection of edifice
Date of dedication of edifice

If a church or a society does not have an edifice of its own at the time the history is presented, a supplementary statement and a photograph may be furnished after a building has been erected and dedicated.
