We're delighted to share with students of Christian Science worldwide the newly designed applications for membership in The Mother Church. They illustrate the prominence Mary Baker Eddy gave to the Tenets, which appeared on the front cover of the earliest application forms.
There are three different forms described in the Manual of The Mother Church by Mrs. Eddy, depending on whether an individual has taken Christian Science class instruction before uniting with the Church. To identify the forms, each one is now printed in a different color.
• Form 2 (green) is for those who have not had class instruction from an authorized teacher of Christian Science. (This form is presently printed in every language into which Science and Health with Key to the Scriptures by Mrs. Eddy is translated.)
